The top U.S. drug regulator urged state officials to help close regulatory loopholes involving the safety of compounded drugs, such as the tainted steroid injections at the center of a deadly fungal meningitis outbreak.

As long as a physician has prescribed a compounded drug, Medicare as well as some private insurers cover it even if FDA has approved a version of the drug from a pharmaceutical manufacturer.
